Optical Policy

LOGIC EYE CARE, INC EYEGLASS POLICY
1. Eyeglasses (frame, lenses) are not covered by Medicare. You can
contact Medicare at: 1-800- MEDICARE (1-800-633-4227)

 

2. Refractive service is not covered by Medicare. This fee ($50) must be
paid even if you do not purchase glasses. REFRACTION: Part of the
examination that determines your eye glass prescription.

 

3. HMO PATIENTS YOU HAVE AN ADDITIONAL COPAY
 

4. A 50% deposit must be paid in order to start your order.
 

5. Orders are completed within 14-21 business days depending on the
amount of orders we receive for eyeglasses.

 

6. Final balance due before we mail out glasses.
 

7. We will provide you with one (1) prescription recheck with in 30 days
of receipt of your glasses. As long as the original refraction service
fee is paid.

 

8. Frames and lenses are guaranteed for 60 days.
 

9. We also offer a 12 month warranty plan ($100) for frames and lenses
which will cover 1 repair call or 1 replacement of lenses or frames.
